How to fix concrete around water meter hole?

My water meter is in the middle of my driveway and is cover by a metal plate which sits flush in driveway.  The driveway is on a slope. As can be seen by the pictures one side is starting to crumble. Do you have any suggestions on how to fix it?

I suspect that water has entered through the split guide of the concrete driveway. When the water got underneath the concrete it softened the soil and when a car drove over it the concrete split and sank. In order to repair it the split piece must be removed and then the foundation repaired before pouring new concrete into its place. 

 

To prevent further water ingress, I suggest using Gripset Betta 1L Concrete Crack Filler. I propose filling in the split guide and the two cracked sides of the broken concrete. This will stop water from going underneath the concrete and making it sink further.
