What tools and materials do I ask for at the store to get this rolling smooth again?

You'd be asking for a plastic drawer runner. If you can measure the thickness of the piece that's remaining, it will help you determine whether you need the Taskmaster 300 x 17 x 13mm White Plastic Drawer Runner or the Taskmaster 295 x 9 x 7mm White Plastic Drawer Runner.
Hopefully, the lugs are spaced the same as your old ones, and you can push the runners straight in. Before the fitment, remove any broken-off lugs that might still be in the holes. If that becomes difficult, you might need to drill them out or drill a new set of holes next to the old ones. This might be a good idea anyway, as the timber surrounding the holes looks to be a bit chewed up, which might be why the runner dislodged in the first place.
If you use the existing holes, popping a little Liquid Nails in each hole wouldn't be a bad idea. That will help glue the runner in place and prevent it from falling out.
