I am looking at replacing this bath tub spout with a hand shower but it won't come off after multiple tries. Is this mounted differently that I am missing something?

I suggest running your fingers around the base of the spout and feel for a small hole. When you find that hole, you'll discover that it has a grub screw inside of it. You'll need an Allen key to remove the grub screw. Once you have the grub screw out, you'll be able to pull the spout out.
However, you'll find attached to your wall spout's outlet is a special spout fitting. In this instance, I suggest engaging the services of a licensed plumber to remove and replace the fitting with a standard threaded pipe. Due to its sensitive nature, removing and replacing the fitting is no easy task. If installed incorrectly the pipe could leak causing damage behind your wall.

Thanks @EricL for the response. I did look for that allen near the base but it is not there.
It is a different spout and tightly attached to the wall.
If that's the case, I'm afraid it's time to call in the plumber. An experienced plumber might have an idea or two on how to dismount the spout from your wall. Worst case scenario is that it will need to be grinded or cut off from the wall.
Let me call on our experienced members @TedBear, @JoeAzza, @Dave-1 and @Nailbag and see if they might have any other ideas on how to remove your spout.

Hi @badmojo @if you can find out the the name of the tap, find out the distributer c then ask them or look up the manufacturer online to locate a spec sheet, it might help, other than that , as @EricL @said you need to engage a plumber to help you.
Hi @badmojo You might have some luck using one of these rubber strap wrenches and grab the spout at the outlet recess to give a better grip and try turn it from there anti-clockwise. These shouldn't damage the chrome work.
And if your going to try a screwdriver at the spout end as @Dave-1 suggests, just first check there is no internal diffuser.

One more suggestion 🙂 You could use a short thick bladed screwdriver and put it into the spout and use that instead of a spanner or shifter. Main reason is less likly to scratch that chrome (broken chrome is sharp) but also to give torque in turning the spout. Like @EricL and @JoeAzza id probarly hit up a plumber first before this last resort. Cracking a tile or damaging the waterproofing isnt worth the pain to fix that as well.

Thanks @Nailbag . I did try a vice grip before as I did on other baths before but this one wouldn't budge. I'll try this rubber grip.
Thanks @Dave-1 . I was thinking actually of just connecting the hand shower to the spout with the diffuser but the thread seems to be non standard. The one I bought from Bunnings that I thought would fit wouldn't fit at all.

I have seen a rubber connection that fits over the end of bath spouts but not sure if they would handle the water from your tap 😕 with the water exiting from the spoute side. This is the style I wa sthinking of Handi Homes Push On Tap Shower
Getting threads to match up can be a problem, you could try a few bolt sizes by hand and see if they wind on but if the spout is from overseas the thread could be different or thread pitch even. No harm in gently trying the bolts tho.
Which one have you tried, @badmojo? I see the aerator thread is recessed within the spout, and that might be the issue.

Hi @badmojo I had a look online searched for a combination mixer and wall spout and found these that might help, the ones I looked at are a push fit, not threaded in place.

I think it was a 20mm thread pipe nipple but it wouldn't fit as it was recessed.
Hi @Dave-1 , I tried a similar one but the challenge is it is on the side and the water would just leak out.
