Hi, I’ve removed an old phone jack from my concrete wall and wanted to know how to patch it. The hole is about 10cm x 5cm.

Welcome to the Bunnings Workshop community. It's sensational to have you join us, and thanks for sharing your question about your telephone jack.
I suggest having a quick look at this discussion - How to remove old telephone ports? by @JonnyH. In the discussion, it was emphasized that the telephone cable be properly terminated and made safe by a proper cabler. Once that has been accomplished you can then cover the phone outlet.
I propose wrapping the cable with a clear self-sealing sandwich bag and securing it with a cable tie to protect it from moisture. I then suggest covering the opening with a Carinya 200 x 60 x 1mm Galvanised Flat Plate Make-A-Bracket. It will need to be trimmed down to cover the opening, you can then render over the opening that's been covered up.

Since the repair spot is not that big, you can use a small bag of mortar to patch hole in the wall.
