If you can provide some dimensions, specifically the diameter of the vent, that would help us identify a potential replacement.
Based on looks alone, the https://www.bunnings.com.au/haron-international-200mm-round-jet-diffuser-ceiling-vent_p0811075 or https://www.bunnings.com.au/haron-international-150mm-round-jet-diffuser-ceiling-vent_p0811074 could be suitable replacements.

@JacobZ Thanks , Jacob. The hole diameter is 130 mm. Could you let me know where the 150 mm measurement was taken?
150mm is the diameter of the duct attachment section at the back of the vent cover. The vent face is 235mm and the required hole size is 205mm, so it would not work with the existing hole. You could still use it, you'd just need to enlarge the hole, which isn't that difficult to do assuming there are no ceiling joists to interfere.
This https://www.bunnings.com.au/haron-130mm-round-snap-in-ceiling-vent_p0811077 is a slightly different design, however it is designed for a 130mm hole.
If you just need a cover, then I'd use the 130mm snap in vent, but if you need to connect to ducting, the 150mm vent is likely the better option.
