How to obtain Form 15 for Mondella shower screen?

Hi, I am in Brisbane and I recently bought a Modella Shower Screen to fit into my newly built bathroom, my certifier requested me to provide form 15 of the shower screen as a part of the requirement to certify the whole bathroom build. I have been to bunnings a few times trying to get form 15 but all failed, could anyone from the community here help? Much appreciated!

Mondella shower screens are imported by a company called Kinetic Sourcing Group, which has provided the relevant documents here - Form 15 - Compliance Certificate & Glazing Certificate - Kinetic Sourcing Group.

Thank you for you reply! 

I have tried the link you provided and seems like it only has form 15 for the glass panels not the Modella product itself, do you know where I can access that? I have tried to call the supplier but no one is at the phone

The response I have received from Kinetic is that the Form 15 on their website, which certifies the glass in their product, is all that is required.

If you provide this to your certifier, it should have everything you need.
