How to fix leaking outdoor tap?

Hi, can someone please give me some advice on how to stop the outside tap leaking. Have replaced the washer and O ring. What have I missed?

Whereabouts on the tap is the leak coming from?

 

Is it from the spout itself, or at the back where it connects to the pipe?

 

If the issue is coming from where the tap connects to the pipe, because it is a connection between a fitting and a pressurised water line, it would technically be considered plumbing work that requires a licence, so you would have to contact a plumber for their assistance.

 

They would likely turn the water off at the metre, unscrew the tap, clean up the threads, removing any thread tape that was used, then apply new thread tape and tighten the tap back in place, ensuring the connection is tight.

 

If the issue is coming from the spout, then you should inspect to see that the washer and o-ring are seated correctly and that there is no sediment in the groove they sit in. Any small misalignment or gaps can lead to a leak.
