Can this grate can be replaced? If, so how?

In order for the grate to be replaced properly, the grout will need to be removed and any adhesive or mortar that is attached to the waste will also need to be removed. You'll need to engage the services of a plumber to disconnect the waste from the waste pipe. They will also reconnect the new waste into the waste pipe. The new waste will need to be anchored back into place with mortar and adhesive then it needs to be regrouted back.
Another option is to paint the waste just like in this discussion - How to paint a shower grate? by @viide. It's an affordable solution to revamp your shower waste.

You could potentially replace the center ring only, tho your grate seems to have rust on the chrome so it may still look not 100% @EricL's suggestion would bring it all back to new and probarrly be nicer underfoot.
To replace the center ring ive actually used and old toothbrush, push the bristells down through a hole, wedge the shaft of the brush against the edge of the hole and pull straight up. With your grill it looks a little small, you could gently use a couple of screwdrivers to pry it up as well. Lie one screwdriver down so the handle is next to the grill but not on it. Then the other screw driver, the shaft goes in a center hole and over the handle of the lying down screwdriver, push down gently, hopefully you will feel and see movement upwards, work around the hole so it keeps rising.
Once out, take it with you to Bunnings and have a look to see the sizing matches with one on the shelf.
