Sorry about the gross picture. I have a 7 year old bathroom/shower. The drain cover recently came dislodged. However I notice a hole extending underneath an adjacent tile. Is this worth a diy repair or should I get a plumber to have a look?

Those voids under the tiles are not a problem at all. They are part of how the shower is built. Water seeps through the grout and is caught by the waterproofing membrane beneath the tiles, then it is channelled toward the drain. For that reason, you should not try to fill those spaces.
What you can do is give the drain area a thorough clean. Clear out any debris and scrub the tops of the small cement pads in the four corners because that is what the grate sits on. Once the area is clean and dry, apply a generous amount of wet area silicone to each of those four corners, then press the drain grate back into position. After the silicone cures, the grate will be held securely so it will not drop out again. If you like, you can run a thin bead of silicone between the grate and the surrounding tiles, although it is not essential. The main goal is simply to fix the grate back in place, and silicone is the easiest way to do that.
